Measure satisfaction and learn why

Ask the score first

One easy question gets a response started. People who answer one question usually finish the rest.

Branch on the answer

Use logic to ask detractors what went wrong and promoters what they value. Each person sees only questions that apply to them.

Keep it to a minute

Three to five questions is plenty. Long surveys get rushed answers or none at all.

Track it over time

The analytics tab charts NPS and every question, so you can see whether changes you make are working.

Customer feedback form: common questions

How is the NPS score calculated?
Respondents rate from 0 to 10. The percentage of detractors (0 to 6) is subtracted from the percentage of promoters (9 and 10). Fomli calculates it for you in Analytics.
Can the survey change based on answers?
Yes. Conditional logic can show or hide questions and jump to different pages depending on any earlier answer.
Can answers be anonymous?
Yes. Simply leave out name and email questions. Fomli never stores respondents' IP addresses.
